Подскажите сколько стоит оформление детского проездного документа и сроки исполнения?

Сам документ стоит ерунду (порядка 100 грн), но для его оформления необходимо нотариальное разрешение родителей (да-да не на выезд ребенка, а всего лишь на выпуск такого проездного документа), а оно стоит в районе 300 грн (+-). Проездной действителен 3 года.
